Great light, description incorrect
Great light, comes with two rail attachment pieces, a two-lug and a single-lug in case the spacing of your rail is different. Comes with batteries. This light is SUPER bright, and seems very well-made and sturdy. The description is WRONG, the switch is on the tail cap, NOT on the side. I specifically bought this model because it says it's on the side. The forward point of my sling could accidentally turn on the light if the switch were on the rear. Which it does. I made it work by putting the light further up the rail than I wanted, didn't want to go through the hassle of returning it.

I was STUNNED AND BLINDED!
Let me start of by saying, this was a super quick set up, got the batteries in before I mounted it, made A HUGE mistake of looking at the lens when I turned it on. I use a 2500 lumen long range adjustable taclight often, this light puts it to shame. If your looking for light weight super bright and illuminating everything you need to see, buy this. Pros: Extremely bright, and a very wide illumination Cons: None

How many slots are needed?

This will mount to a short 3 or 4 slot picatinny rail, but it will take up more space on the rail. The overall length is 3.5".
