A good option for a key-mod rail.
I purchased this for a key-mod optics rail for an AKM, unfortunately the optics mount was a bust so this was turned into a forearm rail for a tactical laser/light.

Is this product a alternative to Diamondhead 2" VRS Threaded Short Rail #2821? Looks like a plastice alternative to the OEM 2821.

It is not quite the same but very similar, yes
what does the back side of this rail look like? From the pictures it seems as though the rail piece sits up off the hand guard rather than mounting flat on the hand guard, is this the case? or is the back of the rail piece flat?
Answer:
It potrudes from the rail about 3/8th of an inch so the Keymod can interface with the rail properly.
