Will this fit a 500 12 Ga with a 6 3/4 slide tube?

Yes this should fit. This item comes with multiple adapters to help it fit different models. However, there are few Mossberg 500 variants that do not have a removable forend and if your forend is not removable, then this will not fit.
Does this fit the Maverick 88 that has the slide rails molded in the grip? The instructions are very unclear. I own it and I cannot figure out how to get it installed.
